Since its introduction to the marketplace in 2006, Chantix (varenicline), Pfizer Inc.’s FDA-approved drug for smoking cessation, caused an alarming variety of side effects. These risks range from nausea, heart rhythm disturbances, seizures and muscle disorders, loss of consciousness, vision disturbances and diabetes to suicides, suicide attempts, behavioral changes, depression, violent outbursts and Stevens-Johnson Syndrome, a rare life-threatening systemic disorder that affects the skin and mucous membranes. After its approval, the FDA soon noted the potential for serious neuropsychiatric incidents in patients taking Chantix through the Adverse Events Reporting System (AERS). The reports compelled the FDA to require Pfizer to provide a mandatory medication guide for patients describing this risk in 2008 and led to the mandated “black box” warning on the Chantix label in 2009 directly relating the association of Chantix with suicidal ideation and aggressive behavior. In 2010, the FDA learned that a few manufacturers, including Pfizer, submitted adverse events reports to the FDA improperly. As a result, serious adverse event reports for Chantix were included in an approximate 26,000 records of non-serious side effects dating back to 2006. The FDA has since clarified its instructions to drug manufacturers, including Pfizer, on the proper channels for the submission of adverse event reports. After Pfizer reviewed its records and properly reported adverse events, the number of high-risk incidents nearly doubled. The FDA continues to monitor serious and unexpected events associated with Chantix and required Pfizer to conduct a large clinical trial on the safety of Chantix as compared to other smoking cessation aids. The federal watchdog itself initiated two observational safety studies on Chantix with the Veterans Administration and the Department of Defense.
